The Sub-Acute Rehabilitation unit (SAR) at Madonna Rehabilitation Hospital serves people with orthopedic and/or medical diagnoses, helping them regain the skills necessary to return to community or other settings of lower intensity.
Licensed as a skilled nursing facility and accredited by CARF, the Rehabilitation Accreditation Commission, SAR is a 17-bed unit located on the garden level of the hospital. A majority of Madonna's Orthopedic patients with hip and knee replacements are served on this level of care.
To be eligible for admission to the SAR, the patient must have a daily skilled nursing or skilled rehabilitation need which would require at least one of the following:
1. Nutritional support of a feeding tube with potential for discontinuation
2. A single IV medication administered not more frequently than every 6 hours through a peripheral IV or PICC. Subclavian central lines considered on a case by case basis.
3. Management of Stage II wounds with simple treatments no more frequently than twice per day, no specialty bed. Postoperative skin grafts or flaps with a need for specialty bed and/or wound vac considered on a case by case basis.
4. Nursing observation/management and respiratory therapy for aerosol treatments not more frequently than four times per day, oxygen less than 4 liters per minute, and/or administration of metered dose inhalers. Patients may have no more than one of these respiratory treatments. A combination of respiratory treatments and/or respiratory therapy treatments required more frequently than four times per day will be considered on a case by case basis.
5. Chemo/radiation therapy (regardless of where provided) with one other qualifying criterion considered on a case by case basis.
6. A MRH established dialysis program with one other qualifying criterion.
7. Need for short-term rehabilitation following general cardiovascular surgery that does not qualify for CARES program (e.g. abdominal aortic aneurysm)
8. Need for physical restorative services of physical, occupational and/or speech therapy less than 3 hours per day
9. Completion of MRH hospital program with one other qualifying criterion evaluated on a case by case basis
10. Supportive care of the dying evaluated on a case by case basis.
Patients on the SAR at Madonna receive an average of one hour of therapy per day, five days per week as well as skilled nursing care. While Madonna follows the same transdisciplinary rehabilitation approach described in the ARU section, patients on SAR may either require or be able to tolerate a much lower intensity of rehabilitation due to medical needs or other factors.
